- Code of Conduct Form – A Code of Conduct Form is required on file One Per Gym!
- Release/Waiver Form – Form is Required for each participating participant.
- Credit Card Authorization Form “Coaches complete this form if you are paying using a CC”. This Form MUST accompany Registration Form when Faxing. For your security, please do not email this form. Complete and return by Fax or Mail.
